LA middle school students performing brain surgery? Well, virtually.

Nearly 130 middle school students held real surgical instruments, looked into the DNA of bananas and conducted virtual brain surgery. it all happened at the Brainworks one-day program sponsored by the Cedars Sinai Department of Neurosurgery.

“They show you how to take out a brain tumor from, like, the head and how you have to use precise tools to do it,” says aspiring doctor Mike Hewitt, an 8th grader who attends Johnnie L. Cochran, Jr. Middle School in south Los Angeles. those precise tools include a real-life 3D-imaging machine and a $500,000 microscope that the kids used to conduct tumor removal from a phantom skull.

At another experiment station in the same room, Victoria Guardi and Ariah Johnson, both 8th graders at James A. Foshay Learning Center, ranked the plastic tub of sheep brains as their favorite part of the event.

“Because you actually get to touch it and you get to see what’s really on the sheep brain,” says Guardi. “And it gives you and idea of what you’re brain is like, too, because it’s similar,” added Johnson.

The students, from several lower income Los Angeles schools, quizzed the five attending brain doctors with such questions as:

“How exactly to brain aneurysms form, exactly?”

“Does the brain feel pain during surgery?”

“How much does the human brain weigh?”

And the burning question of the day? “How much money do doctors make in a year?” that query from a female student in the audience drew a round of laughter and applause – but no clear answer.

The students, too, were quizzed in a round of Family Feud, Brainworks-style, featuring neurosurgeon, Dr. Chirag Patil as host.

“First question,” says Patil, “name three parts of the brain.”

About a dozen kids raised their hands and the team got the three answers, cerebellum, brain stem and cerebrum, correct, causing Patil to proclaim: “We’ve got neuroscientists in the room already.”

“What we want to do is expose them to the wonders of science, the wonders of medicine, says Dr. Keith Black, founder of the Brainworks program and chairman of the Department of Neurosurgery at Cedars-Sinai Medical Center. “To let them understand how much fun we have and how much joy we have each day coming to work, being physicians having opportunity to help people, opportunity to potentially save lives.

Black says it’s his hope the Brainworks event will someday become a nationwide program that will encourage middle school students everywhere to consider a career in science and medicine.

Breast implants: NHS bill already in six figures

Nearly 3,000 women have been seen by NHS specialists, of which over 1,000 have had scans and 67 have opted to have the implants removed, according to Department of Health statistics.

So far only 13 patients have actually had the implants – found to contain industrial-grade silicone – ‘explanted’ by the NHS.

But private clinics were responsible for putting in the implants, made by fraudulent French company Poly Implant Prothese (PIP), in 12 of those cases.

The cost of surgery alone for the 67 former private clinic clients who have resorted to the NHS for removal is likely to surpass £100,000.

When the other costs are added up – at least 2,860 GP appointments, the same number of consultant appointments, and over 1,100 scans – the total bill to the taxpayer so far could approach £500,000.

The figures show only about one in 15 former private clinic clients who have undergone NHS scans to see if their implants have ruptured, has opted for NHS removal.

Anne Milton, the Public Health Minister, claimed: “This appears to show that these women are getting the reassurance they need from speaking to an expert or having a scan.”

However, Fazel Fatah, president of the British Association of Aesthetic Plastic Surgeons, (Baaps), disagreed, describing the low removal rate as “entirely unsurprising”.

“It’s clear that only very few are opting for removal without replacement,” he said.

Taking out implants and not replacing them would leave breasts “smaller and droopier” than before the initial operation, he explained.

For women who had opted for enlargement in the first place because of a problem with their own body image, this would be unbearable, he argued.

“The statistics demonstrate that the great majority of women with PIP implants will have to depend on the private sector for the replacement of these faulty devices,” he said.

“There may be some who prefer to leave them in for a time while they decide on their options, but they’re unlikely to be many.”

The Department of Health has sought to keep costs down by only offering removal on the NHS.

Although Andrew Lansley, the Health Secretary, said last month that women with PIPs would be able to get them removed for free even if they had not ruptured, he made clear replacement was never going to be part of the NHS offer.

Mr Fatah added: “These figures show that a significant proportion of patients have not had the help and reassurance they need – and deserve – from private clinics.”

But Sally Taber, director of the Independent Healthcare Advisory Services, responded: “The constant implication that private clinics are refusing to support their PIP patients is unfair and untrue.

“The issue is not a lack of desire to help but a lack of resources – they are struggling with the number of patients that are affected by this situation that was caused by a fraudulent manufacturer of implants and overlooked by all the regulatory bodies in Europe, including the MHRA.

“From the outset private clinics have been contacting all PIP patients as a priority, advising them and doing their utmost to work towards a solution.”

On Friday morning Sir Kent Woods, chief executive of the Medicines and Healthcare products Regulatory Agency (MHRA) reiterated the organisation’s stance that PIP implants appeared to be no more dangerous than those from reputed manufacturers.

He told BBC Radio 4′s Today programme that the rupture rate for PIPs was “not significantly different” from two leading brands, which had rates of 10 to 15 per cent over 10 years. He also said results of toxicology tests in the UK, France and Australia were “reassuring”.

However, he agreed that women should be able to have them out on “compassionate” rather than “risk / benefit grounds”.

Festoons Eyes

The area of your face surrounding your eyes is the first area to show signs of aging. Correction of eyelid hooding and removal of bags and dark circles under the eyes using a technique called Blepharoplasty is one of the most frequently performed Plastic Surgery operations in the United States.

many people however have excess puffy skin on the cheekbone just beneath the eyelid, called a festoon or malar bag, which a blepharoplasty or face-lift will not correct. in fact, after a lower eyelid blepharoplasty, festoons can actually look worse, because after the removal of the puffiness of the lower eyelid above the festoon, the festoon stands out and is much more noticeable.

Festoons, or malar bags are a very difficult problem to solve. the most common complaint about festoons arises after a blepharoplasty has been done to remove puffiness or bags from the lower eyelids. following a lower eyelid blepharoplasty the festoons often look worse, and patients notice them for the first time. Direct excision or surgical removal is the contemporary standard however the scars are unacceptable. there have been experimental results with laser resurfacing using the CO2 laser-but the recovery requires 3-4 weeks of redness and a difficult recovery. in addition although the festoon is tightened, long term the treated area is hypo pigmented, of a lighter color than the surrounding untreated skin A newer method we are using and have had encouraging results with is “skin tightening” with Infrared or radiofrequency energy-there is no down time but 4-5 treatments are necessary spaced 4-5 weeks apart and a final result takes 6 months.

there is a new Radiofrequency skin-tightening machine, the Aluma that is designed to allow us to treat this specific small area of the face.

If you are considering facial or eyelid rejuvenation be sure to check for the presence of malar bags or festoons. If you have festoons and undergo a blepharoplasty there is risk that the festoons will look more prominent. Be sure to discuss the possibility of skin tightening or other treatment of the festoon with your plastic surgeon.

Syria uprising is now a battle to the death

In the heartland of the uprising against Bashar al-Assad a grinding war of attrition has now become an unforgiving battle to the death.

The Free Syria Army has held this territory of orchards and farmland since September, during which time loyalist forces have never been closer, nor seemed more menacing. as rockets regularly thundered on Thursday into towns that residents could neither defend nor leave, the three months of freedom they had savoured now seemed illusory.

There is little left in the town in which the Guardian was based on Thursday, or in the equally deprived and forsaken villages that dot the hinterland near Homs. Electricity here was switched off two months ago, the phone lines were downed last week. And on Wednesday, contact by road was cut with Homs, Syria’s besieged third city, whose fate is seen as a dire warning of what lies ahead for the rest of the area.

Homs was on Thursday a very difficult place from which to flee. only three seriously wounded residents are known to have made it out of the devastated opposition held sectors of the city into the relative safety of nearby Lebanon. Two of the wounded are unlikely to survive.

The rest face a desperate plight, barricaded in concrete homes that are crumbling in the face of the relentless onslaught now spreading to nearby farmland and villages. some residents of this town say a small number of families from the heaviest hit areas of Homs, Baba Amr and al-Khalidiyeh, have managed to hole up in other areas of the city. however they can no longer speak to those left behind, who they now fear face a gruesome fate.

“We’ll be next,” said a doctor at a makeshift medical centre in the heart of this town. The doctors and nurses on duty here had fled the state hospital, one kilometre away, and set up a triage centre and a surgical ward in a derelict house. All day they were tending to dead and seriously wounded men, many of them members of the badly outgunned rebel army.

The patterned plastic sheets the medics had placed on the floor were slick with blood and iodine as more and more war wounded were brought in by their colleagues.

One hulking man in military fatigue pants was carried in on a stretcher with a gaping wound in his navel. “He’s a first lieutentant,” said one of the clinic’s nurses, Abdul Karem, who like everyone else in this overwrought hub, doubles as a revolutionary. The seriously wounded officer was taken to the improvised operating room, as nurses outside prepped themselves for surgery by washing their hands with kerosene and water.

Among those tending to him was an old French surgeon, a veteran of conflict zones dating back to the Vietnam war, who arrived in Syria on Thursday with a suitcase of medical supplies and a readiness to stay as long as he’s needed.

The carnage of the rest of the day suggests he may be here awhile. Minutes after the lieutenant’s treatment began, a truck screamed to a stop outside and Free Syria Army soldiers bellowed for a stretcher. The triage centre rapidly emptied, as the medics inside grabbed their flip flops – one also reached for his Kalashnikov – and hurried into the courtyard outside. they stopped next to the truck and looked inside and visibly stopped in their tracks. “Finished,” one man said. “Take him to the graveyard.” The dead man was a major, the leader of the Free Syria Army in this town, and one of many wounded by an attack on an outpost not far from here.

As night fell, the numbers of dead and wounded appeared to increase. every massive boom in the near distance seemed to herald the arrival of more patients.

“They’re coming from the hospital that we ran away from,” said one medic, Dr Qassem. “It’s only a kilometre away.”

Regime snipers were also wreaking havoc from a nearby intersection on the road to Homs. Opposition forces, meanwhile for the most part watched from hideouts in apricot and peach orchards and farm-houses dotted along muddy brown laneways.

More wounded were brought in, a rebel shot in the hand, another two with bullets in their back. The television showing footage of the carnage in Homs had by now been switched off as the triage room swarmed with walking wounded, frantic medics and others taking refuge from the shelling.

The first lieutenant inside was fading fast. as other surgeons piled the patient’s intestines onto his stomach, Dr Qassem, who was holding a lamp over the operation said: “They are coming for us now. It is going to be very bad.”

And then he added an optimistic note to a day that had so far offered nothing but misery. “The vote at the UN could be good for us in the future,” he said. “All our students and doctors study in Russia and the standards are not good. “All our factories have Chinese equipment and it’s the same thing. if we win, things will change, God willing.”

He switched back to the dying patient as attention switched to the newest casualty, a man shot in the wrist, his blood streaming over shoes piled at the room entrance.

“There have been more than 100 people killed today,” said one young university medical student as he held an x-ray machine over a patient lying prostrate on the floor. “We all have family in Homs and we are very worried about the situation there. It is much worse than here.

“Every day it has been getting worse here and there. no one is coming for us and we accept our fate.”

Early in the day, a re-supply – of sorts – did arrive for the rebels; three sacks of rockets and rusting mortar tubes. they too were brought into the medical clinic and stored out of sight. It was hardly an arsenal to embolden a clearly struggling rebel army, but it was a sign that some weapons are finding their way across the porous Turkish and Lebanese borders.

“These are old,” said one young fighter. “But they will do. We are grateful for everything that we get.”

Surgery Without Anesthesia For Cosmetic Procedures

You don’t have to turn into a zombie for this type of plastic surgery – but it might help

Surgery without anesthesia in ancient times

Although the bizarre moniker twilight surgery has eerie ramifications of Rod Serling’s popular Twilight Zone television program of yesteryear, it actually is becoming an increasingly popular way to undergo some cosmetic procedures – without the administration of general anesthesia.

It might sound scary or perhaps even a little bit shocking, but in reality the benefits of twilight surgery are numerous. In fact, if you think you can handle the idea of a plastic surgeon cutting off part of your face, manipulating the bones in your nose or shaving off some ear cartilage while you’re wide awake, this could be the surgery for you.

Why Twilight Surgery?

Twilight surgery, which originated in the United States but is now becoming popular virtually worldwide, is carried out while patients are awake, although they are put under heavy sedation before going under the knife. In some cases patients wake up having no recollection of the procedure; in others, they remember every nip, cut and tuck.

How it works is simple. Instead of administering a general anesthetic, the surgeon uses a local one along with a sedative, one that makes the patient feel comfortably drowsy and relaxed. this allows him or her to talk during the surgery, in some cases, and actually move and be responsive to the surgeon when required.

Also known as twilight sleep, conscious sedation or monitored sedation, twilight surgery is not for the squeamish, even though both the local anaesthesia and sedative are administered by a qualified anesthesiologist. The latter may include nitrous oxide (laughing gas) or intravenous medications, and for patients who do not want to run the risk of general anesthesia, the benefits are obvious.

General anesthesia can be dangerous for certain people, such as heavy smokers, those who are dangerously overweight or those with certain medical conditions. Often patients worry that they will be put under and never wake up. When it comes to elective surgery, which plastic surgery more often than not is, twilight surgery can take away a considerable risk.

Local anesthesia as opposed to a general is beneficial for other reasons as well. for procedures such as a facelift, twilight surgery will allow patients to smile, laugh and frown during the course of the operation, so that the surgeon can check the movement of facial or wrinkles and, ultimately, achieve better results. with liposuction, the surgeon can ask the patient to stand up so her or she can access the result of their handiwork once gravity has taken its toll. this would not be possible under heavier sedation.

Types of Twilight Surgery

As far as plastic surgery is concerned, the consensus is mixed. Most surgeons would agree that facelifts, tummy tucks, eye lifts, ear pinning and nose jobs, also known as a rhinoplasty, can be carried out advantageously as twilight surgery. Others say that invasive procedures such as a breast enlargement operation can also have good results, while others believe it’s unsafe.

The decision, ultimately, is up to you and your surgeon – but make sure you only use one accredited by the American Society of Plastic Surgeons, or its equivalent, for best results. and as twilight surgery is relatively new, only use a surgeon who has done the procedure before many, many times before. Remember, you only live once – so ensure you do the best to take care of your health while you can, and don’t stint on money when it comes to something like an operation.

Disadvantages of Twilight Surgery:

In some case, twilight surgery is there to alleviate anxiety if a patient has had a traumatic experience while under general anesthesia. so if the thought of being awake during your operation causes you evenmore undue stress, perhaps you should opt for a general anesthetic.

Other disadvantages include.

* In some cases, the patient could move or flinch during the operation, not a good idea when a surgeon is wielding a large knife.
* Patient could suffer a bad experience during the operation and later feel traumatized
* Often patients can feel a minor pulling and/or tugging during surgery, which some may find unpleasant

Advantages of Twilight Surgery:

* The advantages are numerous, although remember that twilight surgery is not suitable for every type of cosmetic procedure.
* Patient can respond to surgeon and move face or other body parts during the operation
* quicker recovery period than surgery performed under general anesthesia
* No complications or difficulties associated with general anesthesia
* Shorter hospital stay, in most cases, as opposed to surgery done under general anesthesia
* No breathing tube required during surgery or after

Many patients say they have less bruising and bleeding afterwards, as well as no post-operative nausea, which is a common after-effect of general anesthesia

Twilight sleep is commonly used during dental surgery with few complications, although some people worry (unduly) that they will reveal some embarrassing secret to their dentist while they are under, or will do something they will regret. Remember that everyone has a different reaction to specific medications, and that a true professional will not even notice if you act a bit more brazen than usual – they may even enjoy it

However, twilight surgery is not for everyone, especially the squeamish or those who experience deep anxiety when even contemplating an operation. But if your plastic surgeon believes you will have better results with twilight surgery, as in the case of a facelift, nose job or liposuction – or if you have had a bad experience with general anesthesia – this may be a good option for you.

Remember, many common operations, such as eyelid surgery, dental surgery and nose jobs, are done with twilight sedation and local anesthesia. It all depends your own comfort level, as well as the comfort level of your plastic surgeon. if the two of you make a joint decision that twilight surgery is the best plan to follow, then by all means go with it. Just make sure you don’t wake up looking like Rod Serling

To Betsy Rothstein of FishbowlDC, “thank you for asking….” « Gretawire

I suspect many of you read FISHBOWL – DC (yes, I do) but in case you missed it today, I have posted Betsy Rothstein’s article about 10 years of ON THE RECORD at 10pm.  (It is posted at the end of my note.)

Today is the exact day we launched…..February 4, 2002.  Incidentally, right before Feb. 4, 2002, I had just had the bags under my eyes fixed and because I was still swollen, I did still look a bit like the Pillsbury Doughboy, didn’t I?     I got so much media attention for being open about plastic surgery that ON THE RECORD at 10pm got a huge media launch.  Everyone one was talking about it.  It was incredible – even the cover of People Magazine!  Although it was certainly not planned to be a marketing ploy, it ended up being a phenomenal to launch a show.  Even when the critics had something to say about me they mentioned our new show…and mentioning the new show – regardless of what was said – was big for our launch!  It is buzz!  It didn’t matter what they were saying – it mattered that they were saying it and paying attention and telling everyone else about the new show.  Buzz gets everyone paying attention.   No other cable news show has ever gotten a media launch like that and the funny thing?  It did not cost Fox a dime.  It was total luck.  At the time I thought – if only I were smart enough to have planned it to work out this way…but it was just plain luck.

Besides working with great people, and having great friends, I have had lots of luck.  I was extraordinarily lucky to have been chosen by Roger Ailes to anchor the new Fox cable show at 10p.   Frankly, he could have picked a lot of people – there are a lot of good people out there – but he picked me.

I also know and have told ALL OF YOU many times here on GretaWire, you are a GIANT part of the success of ON THE RECORD at 10pm for 10 years straight.  Without you, we would not be successful – and we sure would not be #1 for 10 straight years without you.

And, of course, with GretaWire, I feel like I know many of you and that you are my friends – even on those occasions when you are clobbering me for something I said or did  (and yes, many times I may deserve it.)  But, what is fun is that even when we get into the occasional dust up with each other we all get over it and move on.  I love good strong debate.

After reading Betsy’s article below it occurred to me that I left so much out – there are so many trips and interviews not mentioned.  Israel’s Prime Minister Netanyahu? Mexico? or how about the time in Afghanistan when President Hamid Karzai and Secretary of State Hillary Clinton helped me pick out a tie to buy my husband?  and I could go on and on..but you have been with us for 10 years…so you remember the interviews and times, too.

I don’t want to get too sappy (I really am not the sappy type) but let me repeat one more time: I really do enjoy all of you and thank you for everything.

Here is Betsy’s article in which she asks me several questions:

Plastic Surgery – How Before and After Pictures on the Surgeon’s Website Help

The toll of living a full life is slowly and steadily reflected on our bodies. since these changes can be so gradual, it may be hard to notice as they are occurring. when people finally notice how their bodies have changed over time, many immediately think of plastic surgery to restore and improve their appearance. While the process of aging may be gradual, many people have erroneous ideas about how dramatic a change they can expect from the plastic surgery process.

Before contemplating surgery, it’s important to have an in-depth conversation with your doctor about what to expect from the operation.. when you consider your pre-surgery and post-surgery appearance, it’s essential to bear in mind what can be expected from each procedure.

Many surgeons use a modeling process to change pre-surgery pictures of your face or body and alter the picture to give you a good idea of what to expect from your surgical treatment. Examining the before and after pictures can give you an exact idea of what you should expect.

An important thing to consider after having done the surgical procedure is that the effects are rarely if ever apparent immediately following the procedure. Swelling and redness are common and temporary side effects following any procedure. this may give the false illusion that the surgery had little no effect, or even may have had a negative effect on your appearance.

When considering your appearance after the operation, wait a fair amount of time after the operation before making your judgment regarding how effective the process was. Your doctor should be able to give you an accurate appraisal of how your recovery is going, including giving you a fair assessment on when you can expect to be completely healed.

When you finally decide to compare your appearance before and after the process, it’s essential to consider everything your doctor has told you. when examining the pictures, it’s important to look not just at the area that was worked on, but the body or face as a whole. many surgeries work in the small scale, but end up having the greatest effect on the appearance of the body as a whole.

For instance, while the shape of your nose may seem similar to its previous look, the placement and symmetry of the nose in relation to the rest of the face. you are more than a nose, and your body and beauty is reliant on more than just one body part.

Fact Finder: Cold Laser Therapy and it's many uses

Cold lasers come in different sizes, different powers and the irony in its name, it’s far from cold.

Physical Therapist, Lenny Macrina said, “If I kept it there for a prolonged period of time, I would burn you.”

Here at Champions Sports Medicine on St. Vincent’s campus, cold Laser Therapy is used in rehab.

Cold Laser Patient, Bernice Ashford said, “Lots of pain. Linda asks: 1-10, what was your pain? Bernice answers: 10.”

Bernice Ashford is rehabbing her ankle, fused last August. After adding the cold laser to her therapy, she said, her pain has gotten better.

Ashford said, “When I first had the laser and I had to go back and have x-rays, go see my doctor. you could see it on the film, see the healing in the tissue part of it.”

Fact: cold Laser Therapy is described as low-intensity or low levels of laser light. While lasers used in surgery are around 300 watts in power. Low-intensity lasers hover in the 5 to 500 milliwatts, much less.

Lenny Macrina is a physical therapist at Champion Sports Medicine.

Macrina said, “So this I’ve found has sped up wound healing and also helps with pain, and inflammation, swelling so we’ve had a lot of positive results by using this.”

For the last 10 years or so, cold laser therapy has been used here in the US, in conjunction with your typical physical therapy in order to reduce pain and heal wounds.”

Cold lasers are branching out into other areas like ending allergies to dust, pet dander, pollen and more.

Chiropractor, Dr. William Sopchak said, “What it does it sets up your body’s ability to handle whatever it was that you had a sensitivity to.”

Dr. Sopchak’s patients are swearing by it.

Patient, Anne Kowalski said, “I really like the approach that doesn’t invade the body that gives you an alternative to taking pills and shots.”

And you’ll also find cold lasers being used for cosmetic purposes, as an alternative to plastic surgery.

Last September, Dana Walker turned to Azia Medical Spa and it’s Zerona laser to lose inches off her frame.

Cold Laser Client, Dana Walker said, “I didn’t want the down-time, didn’t want the down time and I wanted to, it was just very easy to get your clothes back on and drive home.”

This is an example of how the cold laser works in this procedure. Let’s take this bubble wrap for instance. We’re going to measure first. it measures about 10 inches around. Now we squeeze the bubble wrap, now this is how the cold laser works, getting fat cells to seep into the body. once you re-measure the bubble wrap, you’ve lost some inches.”

Co-Owner, Azia Medical Spa, Alex Mims said, “It doesn’t interrupt the other organs, the way it’s designed, the science behind it, it does the fat, that’s right underneath the skin.”

The treatment isn’t cheap: 18-hundred dollars for 9 sessions over a three week period.

Fact:  not everyone is eligible, clients are prescreened.  if given the ok, clients take supplements, exercise, avoid caffeine and drink plenty of water.

Walker lost nearly 5 inches. While weight-loss is a side effect, she didn’t experience that.

Walker said, “And also I liked the results, I really do. There’s just some places that exercise, doesn’t, you don’t see the results as fast.”

Fact: cold lasers have also been used in the treatment of glaucoma, spider veins, cancer, psoriasis, and hair regrowth.

As for the use of cold lasers in her physical therapy, Bernice is happy with her results as well.

Ashford said, “If I didn’t have the laser, I think it’d be a prolong healing and returning back to work. I’m looking forward to going back to work very soon now.”

While insurance did pay for Ashford’s cold Laser Therapy, it doesn’t pay every time. you should check with your insurance company. The FDA has yet to approve its use for pain, so insurance companies are reluctant to reinburse for it. it is waiting for more concrete proof, in the way of medical studies, before giving its ok.
