Aimee Copeland Pleads for Painkillers, Feels Like ‘Patchwork Quilt’

Aimee Copeland, the Georgia student recovering from flesh-eating disease, is pleading for painkillers after surgery to replace swaths of bacteria-ravaged skin and muscle, her father said.

Copeland, 24, was hesitant to take morphine, telling her father she felt like “a traitor to her convictions.” But her preferred method of pain management, meditation, proved no match for the sting of skin grafts and muscle flaps to close a gaping wound on her abdomen and groin.

“Aimee is now taking pain medication in as liberal a dose as can be prescribed,” Copeland’s father, Andy Copeland, wrote in a blog post. “If she even dared to refuse taking it, which she wouldn’t, then the doctors would most certainly administer it in an IV drip.”

It’s been nearly seven weeks since Copeland cut open her calf in a fall from a homemade zipline near the little Tallapoosa River, inviting a flesh-eating infection that claimed her left leg, right foot and hands. Doctors also removed part of her torso, leaving a wound that was dutifully cleaned and covered with sterile bandages in advance of reconstructive surgery Friday.

“During the most recent skin graft, her surgeons were forced to take muscle from Aimee’s abdomen to create a flap over the iliac artery in her groin,” Andy Copeland wrote. “She says that she feels like a patchwork quilt, because her body is a collection of skin grafts and bandages.”

A skin graft transplants a thin patch of skin surgically shaved from elsewhere on the body onto a wound.

“We can get sheets between 10 and 12 thousandths of an inch thick,” said Dr. J. Blair Summitt, assistant professor of plastic surgery at Vanderbilt University Medical Center in Nashville, Tenn. “Within two or three days, tiny little blood vessels start to grow into the graft. It’s a fairly straightforward procedure.”

Straightforward, but not painless. Summitt said narcotic painkillers like morphine and Fentanyl help patients power through the painful reconstructive surgery. But Andy Copeland said no drug is powerful enough to relieve his daughter’s pain.

“The allowable doses of Morphine, Fentanyl and Lyrica are often inadequate to deal with the pain that Aimee is now experiencing,” he wrote. “Please believe me when I say that Aimee’s refusal to use pain medication has ceased following her most recent surgery. She is now requesting it ahead of schedule.”

Despite the pain, Copeland’s character shone through as she wished her dad a happy Father’s Day Sunday.

“Sometimes being a parent is not easy and this is one of those times,” Andy Copeland wrote. “I would much rather be the one in that bed. if I could take that pain away from her I would do it in a heartbeat.”

Johnson & Johnson to stop selling surgical mesh implants

WASHINGTON (AP) — Johnson & Johnson plans to stop selling surgical mesh implants used to treat women’s health problems, which have been linked to injuries and triggered hundreds of lawsuits.

The company sent a letter Monday to judges in New Jersey and West Virginia who are overseeing patient lawsuits against the company. J&J said it plans to phase out four mesh products over the next three to nine months.

The plastic mesh is used to strengthen the pelvic wall in cases of pelvic organ prolapse, in which the bladder or other reproductive organs slip down into the vagina. About 75,000 women had prolapse surgery with mesh inserted through the vagina last year. A similar procedure using an incision in the abdomen is less common.

J&J said the mesh products are safe and that it is not recalling them.

“Our decision to discontinue these products is based on their commercial viability in light of changing market dynamics, and is not related to safety or efficacy,” the New Brunswick, N.J., company said in a statement.

But the Food and Drug Administration reported last year that the implants were associated with higher rates of pain, bleeding and infection than traditional surgery with stitches. the agency said 10 percent of women experienced erosion or exposure of the mesh within 12 months of having the mesh implanted. more than half of these women required follow-up surgery to remove the mesh. some required two or three surgeries.

At the same time, the FDA said it saw no evidence that using mesh led to better outcomes than traditional surgery with stitches.

In January, the FDA ordered J&J and five other companies to conduct rigorous studies to track the complication rates with their products over time. J&J said in its letter it expects the FDA to waive that requirement once it phases out the products.

Consumer safety advocates hailed the announcement as a victory.

“These companies know they will never be able to prove safety and efficacy of these devices in the studies mandated by the FDA in January of this year,” said Lana Keeton, a Miami resident who has undergone 17 surgeries to remove mesh that was implanted in 2001. Keeton’s group, Truth in Medicine, has lobbied the FDA on the risks of mesh in recent years.

Mesh products initially were seen as a high-tech improvement over traditional surgery, which also can have complications. since similar mesh was already used in other types of surgery, the products received fast-track approval from the FDA without the tests that the agency requires for first-of-a-kind devices.

In its last quarterly filing with regulators, J&J said the “number of pending product liability lawsuits continues to increase,” and that it has set aside money to pay for litigation costs associated with the mesh, though it didn’t specify an amount.

Federal cases against the company have been consolidated in the Southern District of West Virginia. the state cases in New Jersey have been centralized in a state court in Atlantic County.

Other manufacturers of pelvic mesh include Boston Scientific Corp., Covidien plc, CR Bard inc. and American Medical Systems.

Study: Lipo may not Be a Permanent Solution, but Exercise Helps woman marked for liposuction

June 15, 2012 — Liposuction can help get rid of muffin tops, love handles, and other pockets of fat. but new research shows that removing fat from your abdominal area may cause you to gain dangerous visceral or belly fat.

This type of fat is stored around the organs deep within your abdomen, and it increases your risk for heart disease and diabetes.

That’s the bad news. the good news is that regular physical activity can help counteract this effect. That news appears in the July issue of the Journal of Clinical Endocrinology and Metabolism.

“If someone chooses to undergo liposuction, it is very important, if not essential, that this person exercises after the surgery,” researcher Fabiana Braga Benatti, PhD, of the University of Sao Paulo in Brazil, says in a news release.

Exercise Advantage

The fat — and fat cells — are removed permanently during liposuction. This means there are markedly fewer fat cells in the treated area, and fat regrowth will not occur there.

Fat cells in other parts of the body, however, may pick up the slack.

The new study included 36 women in Brazil who underwent small-volume liposuction of the abdomen. Half of these women took part in a four-month exercise regimen starting two months after the surgery. six months later, women who did not exercise showed a 10% increase in visceral fat, compared with women who exercised three times a week.

The women who exercised three times a week began each session with a five-minute warm-up followed by strength training and up to 40 minutes of treadmill time.

Plastic surgeon Sherrell J. Aston, MD, reminds all of his patients that liposuction is a cosmetic procedure that does not necessarily lead to permanent weight loss. “If you take fat from one area, your body compensates in other areas,” Tiwari says. “We gain weight for metabolic reasons and those hormonal drivers are not changed by liposuction.”

Exactly, says Lisa M. Donofrio, MD. she is an associate clinical professor of dermatology at Yale University School of Medicine in new Haven, Conn. Various studies have documented weight regain in various body parts following liposuction. “This is the first one that talks about regain of intra-abdominal fat,” she says.
