Ask the Plastic Surgeon: Finding the best brow procedure for you

Question: at a consult for upper eyelid surgery, the doctor also suggested I do a brow lift. Why would I need a brow lift instead of just my eyelids?

Answer: As we age, our brow descends from gravity as well as the downward pull of the facial muscles used when we squint and frown. in addition, the fat from behind our eyeballs moves forward with age and settles in our eyelids, thus creating those heavy, puffy lids.

One way to determine (for yourself) if you need the brow lift or just upper-lid surgery would be to look in the mirror and try this: If you like your look best when you lift your whole forehead or pulling the brow (up with your fingers) to get the look you like, then you will probably require a brow lift.

On the other hand, if you find you are satisfied by just pulling the extra skin on your lids forward, then maybe you really only need upper-eyelid surgery (blepharoplasty) for correction.

To determine the dynamics of what procedures are actually needed for upper-face rejuvenation is something I review during a consultation with my patients. at this time, I touch on the pros and cons of doing eyelids and brow together, or separately, and also which type of brow lift may be best suited (endoscopic, traditional, temporal, etc.) to achieve their rested look.

On another note, sometimes it may be just BOTOX® or Dysport® injections that can provide just enough of a brow lift to avoid brow-lift surgery for now. even better for the patient, by doing these injections regularly in the brow may ultimately slow the rate at which the brow will descend over time.
