Dr Raju Raj Pandey's clinic looks like any regular clinic from the outside, however, once inside you are greeted by photographs of Nepali noses, eyes, foreheads and other body parts, before and after surgery. Pandey is a cosmetic surgeon.
All over the world cosmetic surgery is an emerging business. especially in Western countries, celebrities are getting older by looking younger. Noses are shrinking, breasts are growing, wrinkles are vanishing, the skin is tightening, and the tabloids keep us up-to-date about which super star tried out the latest facelift method. In Nepal, however, cosmetic surgery is still in its infancy. “We do about three to four upper eyelid surgeries per week,” says Pandey who is the founder of the Nepal Plastic Surgery Hospital. “In centres in Bangkok they performed about 50 to 100 eyelid surgeries per day.”
Although in Nepal cosmetic surgery is still an unknown concept to many, demand is gradually growing. especially young girls knock at Pandey's door, asking for bigger eyes and smaller noses. “It takes time, but slowly people are coming,” Pandey says.
Plastic surgery can be divided into two broad categories: reconstructive and cosmetic surgery. Reconstructive surgery helps after accidents, for example to treat burn injuries. In Nepali villages, where open fire cooking is common, children playing around open fire often become victims. Reconstructive surgery is sometimes necessary for survival.
Cosmetic surgery, on the other hand, is a surgical intervention based on the patients' wish to modify a part of their body for aesthetic reasons and usually unnecessary for survival.
Insurance plans in Nepal don't cover cosmetic surgery and patients need to pay out of their own pockets. some hospitals financed by aid organisations provide reconstructive surgery for people in Nepal who cannot afford the treatment. the Sushma Koirala Memorial Hopsital in Salambutar, Sankhu, is one example.
The most popular among cosmetic surgeries in Nepal is the 'Mongolian eyelid surgery', says Pandey, who studied medicine in India, earned a master in surgery in Nepal and went to Bangkok for special training in plastic surgery. During the surgery, fat, excess skin and muscle from the eyelid are removed to make the eyes appear bigger.
Dr Jaswan Shakya (pictured left) earned a bachelor in medicine and a master in surgery in Russia and got special plastic surgery training from South Korea. he works part-time at the Nepal Skin Hospital, where certain types of cosmetic surgery are offered. Shakya says Nepalis living abroad in the UK or US come back to Nepal for the surgery. “It is more than five times cheaper here in Nepal than in Europe or the US,” he says.
Costing between Rs 20,000 and 30,000, the Mongolian eyelid surgery is not only the most popular but also the cheapest surgery in Nepal. it is considered a very simple surgery. Lasting about one hour, the patient can walk normally home on the same day.
Botox, in turn, a very popular method in Western countries to reduce wrinkles, is very uncommon in Nepal. Instead of a surgery, it requires the injection of expensive medicine. a botox injection costs Rs 10,000 to 12,000 and needs to be repeated every three to five months. “For celebrities, it is okay, they earn for their faces, but from normal people, it is too expensive,” Shakya says.
Other cosmetic surgeries in Nepal, mainly offered at Nepal Skin Hospital are nose surgery, hair transplantation, face lift, breast implants, ear surgery, scar revision or tattoo removal.
Pandey says the reasons why patients want cosmetic surgery are manifold. those coming for a Mongolian eyelid correction are often young girls around 20 or 22. he says some of them want to work as stewardess and think they need the correction. “New eyelids doesn't mean that they will automatically get the air hostess job,” Pandey warns.
However, he agrees that for some jobs, a little correction can be helpful. “You might apply for a job and the employers might tell you that you are too old for the job, because of your receding hairline. In such cases we offer hair transplantations,” Pandey says.
Some girls even bring pictures of models to his office. “They tell me 'make me look like this model',” Pandey says. “But I usually tell them that it is not possible. Everyone has a unique face structure that we cannot change.”
But it isn't just young girls who are rushing to cosmetic surgery clinics. Men are interested in nose corrections or hair transplantations. Both men and women above 50 perform upper eyelid corrections because of their age. “For them it's functional,” Pandey says.
Based on his experience with patients, Shakya says cosmetic surgery patients in Nepal are still not under as much societal pressure to look a certain way as people in the West. “This is the difference between Europe and Nepal,” he says. “In Nepal they do the surgery out of their own will, in the US or in Europe, they are pressurised to follow certain trends and look like celebrities. but I am sure this obsession to look like celebrities will soon be felt in Nepal.”
Both Pandey and Shakya have had patients who had second thoughts about surgery. Every surgery entails risks such as bleeding or swellings and Shakya says he meets patients at least three or four times where he talks to them in detail about the procedure and side-effects and precaution. “Cosmetic surgery is not an emergency surgery. the patient must understand the situation,” he says.
Shakya recommends his patients to watch videos and read articles about cosmetic surgeries first before making a decision they might regret later on. “If there are complications after breast surgery, we might have to remove the implant, and the patient loses 2000 Euros,” he says.
Pandey also tells his patients to reconsider cosmetic surgery and 25 per cent of them eventually change their mind.
Despite being a plastic surgeon, Pandey still believes in natural beauty. “I believe that it is the nature which makes you who you are,” he says. “You can't challenge nature, you can only modify things.”
A droopy or saggy neck is usually the single most bothersome facial change for men as they get older. known as a neck wattle, this loose skin and fat causes a loss of a once sharper neck angle. this new roll of displaced neck skin can get in the way of shirt collars and is bothersome as it tends to flop around as one moves. such neck wattles are usually not present before age 55 or older unless one has lost a lot of weight at a younger age.
While a conventional facelift is the ideal approach for an aging neck, men are much more sensitive about this procedure. Concerns about its recovery, visible scars around the ears, lack of a good hairline and hair density, where the scars would be, and the fear of being recognized of having had a facelift make many men shy away from its serious consideration.
An alternative to a facelift in the older male is the direct neck lift. rather than working the extra neck skin up and back to the ears (and the subsequent scars), the direct necklift removes the skin in the midline of the neck where it lies. this is a far simpler operation, with very little recovery, no pain other than a little neck tightness, and no change in the hairline or scars around the ears.
The obvious downside to a direct neck lift is that of the scar. rather than it being around the ears as in a typical facelift, a midline scar running down the middle of the neck is created. While the direct necklift is far simpler and even more effective at creating a sharp neck angle, the pertinent question is what does the scar look like and how noticeable is it?
The best answer to that question is to look at actual neck scars from the procedure. In my Indianapolis plastic surgery practice, my experience shows that the scars are quite acceptable. I have yet to have a male patient tell me that they regret doing the procedure and consider the neck scar worse than their original neck wattle.
Why do the neck scars usually turn out quite well? Besides good operative technique, the main reason is the unique healing quality of bearded skin. Men’s skin is thicker, has more blood vessels, and heals better due to the cellular contributions from the hair follicles. In addition, most men do a daily scar treatment after surgery, they shave. this is the equivalent of doing daily microdermabrasion scar treatments for years.
Not all neck scars will be perfect. a few will develop some thickening of the neck scar just above the adam’s apple (thyroid cartilage) where the tension of the skin closure was greatest from the excision. For many men they do not feel that this is a problem. For others I treat it with some steroid injections or perform a small z-plasty in the office to relieve it in the first year after surgery.
In 2011 there were more than 1.6 million cosmetic surgery procedures performed, according to the American Society of Plastic Surgeons.
With so many procedures being performed, plastic surgeons are pointing to the importance of conducting the proper research to ensure patients avoid common mistakes when considering plastic surgery.

Which surgeon is best? Whether you are looking to have a facelift or a simple injection of Botox, you need to compile a list of surgeons in your area. from there you should be able to speak with friends and family members to see who they recommend and set up a consultation.
A consultation does not mean the surgeon you meet with is the surgeon you must have perform your surgery. if you are not comfortable with them or do not feel they have enough experience, move on to the next candidate.
Patients must feel confident in their decision in choosing us as their surgeon. They must be encouraged to research the surgeons’ experience, education and credentials.
Cheaper is better In today’s economy, price shopping is a necessity. however, in the realm of plastic surgery, price shopping is one of the worst ways of picking a surgeon. The doctors who offer cut-rate services are generally not board certified or not good surgeons.
Comparing prices when car shopping is one thing, but when it comes to health matters, patients should never settle for the lowest price. you get what you pay for.
Not entirely comfortable with the surgeon When a patient does not feel comfortable or confident in their surgeon, it is unlikely that they will obtain the results they are looking for. While surgery only takes a few hours, most people forget there are numerous appointments before and after surgery, so a good rapport should start with the initial meeting.
Forgetting to review complications It is important for patients to fully understand the risks and complications associated with the procedure they want to have done. They should not be afraid to ask about the risks and find out where they fall as far as complications.
The surgeon is supposed to go over everything with their patient from pre and post surgery care to consent forms and recovery time.
Not asking the right questions Questions will pop up in people’s minds during the consultation, but there times when they can go totally blank. It is advised to jot down a list of questions to ensure there isn’t a gap in communication.

Linda Grisham says it was time to change her look.
“I didn't like the way that this was all saggy and felt like my neck was sagging,” she said. “And I felt like I was too young to be looking like that.”
Dr. Michael Jasin specializes in facial plastic surgery. in addition to a facelift, he recommended another enhancement.
“The enhancement was a chin implant and immediately in the computer imaging, you look at the difference,” he said.
And that means more people are comfortable getting the implants, according to Dr. Jasin. He's seen a huge spike at his Tampa office.
“I happen to have a chin implant myself,” said the doctor.
Chin augmentation is the fastest growing plastic surgery trend in the country, according to the American Society of Plastic Surgeons. it went up 71 percent last year, and the numbers are even higher among men.
“People are now seeing their facial images more frequently than they normally do,” he explained.
Most of the patients are baby boomers. they can thank technology for pointing out their “not so pointy” chins. Experts say social media, video chatting, Skyping, and camera phones have shown us a side of our face that we don't necessarily like.
“A chin that recedes will make a nose look bigger. it will make a neck look less defined and it will make a jaw line look less defined,” said Dr. Jasin.
So getting a chin implant can actually make your nose look smaller.
“It's done with a local anesthetic. it takes about 30 or 40 minutes to do the procedure. There's minimal down time and it can produce a very, very natural yet enhanced look on your face,” he said.
“I had no discomfort, it doesn't move, it doesn't hurt, it's just like a part of me,” Linda said.
There are some risks, though.
“Implants can come out or migrate,” Dr. Jasin explained. “But it's very rare.”
“The difference was just a more youthful look, more prominent, I didn't realize that when you age you lose your chin, I don't know, I didn't think about it,” Linda said.
Even her stepkids were impressed
“They said, dad's a lucky man.”
Linda said the implants are making her feel more youthful.
“Sometimes I feel like I'm 15. I have a lot of energy,” she added. “I'm so glad I did it. I wish I had done it sooner.”
