Cosmetic Surgery Risks

Everyone considering cosmetic surgery should always be concerned about the risks of the procedure. whether the procedure is Botox or Juvederm or a more invasive procedure such as a facelift, risks are inherent. some risks span the gamut of procedures and others are specific to certain procedures. As always, you should speak with an experienced and trained surgeon before undertaking any plastic surgery procedure as risks are dependent on the individual, the procedure being performed, and the exact method for performing the procedure.

Bruising and swelling

Any procedure will result in a variable amount of bruising and swelling. While no person can estimate the amount of bruising and swelling, some things can be done to decrease the amount. Avoid medicines which prevent blood clotting such as Aspirin and Ibuprofen. If you are taking prescription strength aspirin or other prescription medications that prevent blood clotting, speak with your doctor about stopping those medications for a few days around the time of the procedure. be careful of over the counter supplements and vitamins as some of these can also increase your risk of bruising.

Poor cosmetic result

Every cosmetic surgery procedure is done to try to improve the way something looks. unfortunately, some procedures end up causing things to look worse or maybe not quite the way things were expected. be sure to convey to your plastic surgeon anything that bothers you about your results. often times, a simple revision procedure can be undertaken which corrects the problem.

Scarring

Anytime the skin is broken, a scar will result. A skilled cosmetic surgeon can hide these scars in areas where they will not be easily seen. Scars can take quite a while to fully heal. be sure to abide by the surgeon’s recommendations for scar treatment. be careful of sun exposure as this can lead to either light or dark scars.

While these risks are common to almost any cosmetic surgery procedure, be sure to sit down with a trained and experienced plastic surgeon before undertaking any procedure. Disclose to your surgeon all medications, including over the counter and herbal supplements you may be taking. Every procedure has its own risks and a skilled cosmetic surgeon can speak with you about them so that you can make an informed decision about any plastic surgery procedure.

Scoliosis Surgery – Medically Necessary or Cosmetic?

Over 38,000 scoliosis surgery procedures (adult and adolescent patients combine) are performed in the United States every year and there is no medical indication for the highly invasive procedure with poor long-term outcomes and a mortality rate (death) equal to that of swine flu (H1N1 virus) [fatality rate for swine flu could be anywhere between 0.1%-0.35% and for scoliosis surgery the mortality rate was 0.17% and 0.40% for pediatric and adult patients.] The primary and only indication for scoliosis surgery is cosmetic improvement. Which, in all fairness is an important aspect and treatment outcome in scoliosis treatment, however current research suggests it doesn’t succeed in the in its attempts to correct the spinal deformity associated with idiopathic scoliosis either.

  1. Scoliosis surgery fails to improve cosmetic spinal deformity in scoliosis. first of all, no seems to think about the massive scar the length of the spine that results from this procedure (that will ruin bikini season) and a recently published study on the topic of spinal fusion surgery and cosmetic improvement concluded, Radiographic and physical measures of deformity do not correlate well with patients’ and parents’ perceptions of appearance. Which is odd, because a 17 year post follow-up study found 40% of the post scoliosis surgery patients were legally defined as severely handicapped.

This is in sharp contrast with the 50 year follow up study of un-treated scoliosis patients who seemed to have an significantly increased quality of life than many of the post fusion patients. Surgery does not improve lung function. a published study in the Journal of bone and joint surgery found

no remarkable improvement in pulmonary function more than two years after surgery; three patients showed deterioration. The results of tests performed less than two years postoperatively showed no improvement in pulmonary function, irrespective of the types of assessments used. Furthermore, an even more recent study from ModernMedicine.com concluded, this study supports the previously published theory that any surgical procedure that disrupts the chest wall has a negative impact on pulmonary function,

It does not necessarily halt curve progression in adults. The average curve progression rate in adults with un-treated idiopathic scoliosis is 1-3 degrees a year. Post scoliosis surgery studies indicate a rate of curve progression in post spine surgery adults at Initial average loss of correction post spinal surgery is 3.2 degrees the first year, 6.5 degrees after two years, and 1.0 degrees every year after that of the course of the patient’s life.

I believe Dr. Paul Harrington, known for inventing the scoliosis surgery that implants metal rods in scoliotic spines, stated in 1963, metal does not cure the disease of scoliosis, which is a condition involving much more than the spinal column. I highly encourage every patient whom is considering any treatment to dig deep into the research available (both pro and con), ask your doctor as many questions as you can think of, and have your x-rays read by at least 2 radiologist (non-surgeons), because they are unbias and the Cobb angle measurement (used to determine the need for scoliosis surgery for some reason) has a inter-examiner measurement error of +/-5-10 degrees. everyone has the right to make a truly informed decision.

Cosmetic Surgery and Its Risks

As popular as cosmetic surgery is, not everyone is properly informed about the risks that are associated with even the least invasive procedure. Failure to take heed and understand the risks that are associated with any medical procedure can often make dealing with any complications challenging. there are risks associated with every action you take, but if you want to increase your understanding and improve your chances for success and optimal recovery, you may want to take the time to learn more about some of the risks that are associated with cosmetic surgery. this list of complications is not to scare you or deter you from cosmetic surgery; it is merely for your knowledge. Keep in mind that these risks are significantly lowered if you have your procedures done by a board certified surgeon, and some of these risks are associated with any type of surgery. make sure your doctor goes over these risks before you commit to any procedure.

Some patients may have an adverse reaction to the anesthesia that is used for the procedure. Symptoms include irregular heartbeat, airway obstruction due to swelling spasms of the respiratory pathways, blood clots, brain damage due to lack of proper oxygen, and poor blood circulation. those who suffer from cardiac issues, lung disease, and obesity are more likely to suffer from complications from anesthesia. this is also why it is extremely important to be honest and forthcoming with your doctor about your medical history.

Less severe complications include numbness and tingling sensation due to nerve damage. often this is only temporary, but in a small percentage of cases, it is permanent. Fluid accumulating under the skin (seroma) often happens after a tummy tuck or breast augmentation. Death of skin tissue (commonly referred to as necrosis) usually comes up after an infection and has been found to be more common in patients that smoke. the affected skin can be surgically removed. Healing that takes longer than usual can be caused by several factors such as age, ignoring your doctor’s orders, and poor skin condition.

If your surgeon has placed you in the high risk category, they will give you very specific instructions on things you should do to help minimize your risk for any complications. if you are determined not to be high risk, you may not have a list of post-operative instructions to follow.

If you want to further reduce your risk for complications during cosmetic surgery, you can ask your surgeon about them taking extra precautions such as elevating your legs during your procedure to increase your blood circulation.

Remember, to follow all your doctor’s orders before and after your cosmetic surgery. Eat healthy, drink lots of water and liquids, and get plenty of rest. Even if you feel great after your surgery, do not take on any activities that would interfere with your healing and recovery. if you are required to take it easy and avoid strenuous activities, then you should obey that order and get someone to help you.

Minimally invasive approach to weight-loss surgery reduces complications, Stanford study finds

BY JOHN SANFORD – a study by researchers at Stanford University Medical Center has found that a popular weight-loss operation is safer and reduces hospital bills when done with minimally invasive techniques rather than open surgery, which requires a large abdominal incision.

The authors say that, to their knowledge, this is the first time the open and minimally invasive approaches have been compared at a national level. “There have been single-center randomized trials that support the greater safety and efficacy of the minimally invasive approach, but what our study does is to confirm that those results are actually occurring in practice at hospitals and academic medical centers across the country,” said John Morton, MD, MPH, associate professor of surgery at the School of Medicine and senior author of the study, which was published June 18 in the Archives of Surgery.

The patients who underwent the laparoscopic, or minimally invasive, procedure had lower mortality rates, lower complication rates, shorter hospital stays and lower hospital charges compared with those who underwent open surgery, even after adjusting for differences in the patients’ socioeconomic levels and co-morbidities, the study reports.

The researchers examined 156,271 Roux-en-Y gastric bypass cases performed between 2005 and 2007 nationwide, including 115,177 patients who had the laparoscopic approach and 41,094 who had it done as open surgery.

The operation involves making a small pouch from the upper part of the stomach and connecting it directly with the middle section of the small intestine, bypassing the rest of the stomach and upper section of the small intestine. the result is that patients cannot eat as much food at one time and feel fuller with smaller portions.

Morton, who is also director of bariatric surgery at Stanford Hospital & Clinics and director of the Surgery Center for Outcomes and Research Evaluation at Stanford, said he wasn’t surprised that the laparoscopic technique got good marks for patient safety. “What did surprise me was the degree of superiority pretty much across the board compared with open surgery,” he said.

There were lower rates of in-hospital complications experienced by the laparoscopic patients in 15 out of 18 complication categories, including cardiac arrhythmia, sepsis and the need for a blood transfusion, the study reports. in addition, the patients who had the minimally invasive procedure were found to have shorter hospital stays.

The authors cite previous research in noting that “mortality and other complications are serious risks associated with bariatric surgery and are inversely correlated with the volume of cases performed by the surgeon.”

They add: “The Centers for Medicare & Medicaid Services determined that bariatric procedures should only be performed at high-volume facilities that are either certified by the American College of Surgeons or the American Society for Metabolic and Bariatric Surgery/Surgery Review Corporation as a Bariatric Surgery Center of Excellence.”

The Centers for Medicare & Medicaid Services define high-volume practices as those that perform 125 or more weight-loss surgeries annually. Approximately 300 weight-loss operations are performed each year at Stanford Hospital, whose Bariatric and Metabolic Interdisciplinary Clinic is the is the only Bariatric Surgery Center of Excellence in Northern California to achieve Level 1A certification by the American College of Surgeons.

The study’s lead author is Gaurav Banka, MD, a former Stanford medical student. other co-authors of the study are Gavitt Woodard, MD, a former Stanford medical student, and Tina Hernandez-Boussard, PhD, MPH, co-director of the Surgery Center for Outcomes and Research Evaluation. Chin Augmentation Gains Popularity Among Professionals

Chin augmentation is a cosmetic procedure designed to define and enhance the jaw line that most people are availing these days. in fact, its popularity soared in just one year, making it more popular than breast augmentation, Botox injections and liposuction. according to the American Society of Plastic Surgeons (ASPS), 10, 593 males and 10,087 females underwent the procedure in 2011, showing a 76% and 66% rise over the past year, respectively. this statistics proved that chin augmentation outdid lip augmentation, cheek implants and facelift which only showed 49%, 47% and 5% increase, respectively.

This recent trend can be puzzling, especially since Botox injections are still the most talked about non-invasive procedure offered by most clinics but actually, it has logic to it. in a report made by the ASPS, chin implants are more popular among patients over 40 years old. the reason behind is that, the jawline and the chin are usually where the first signs of aging are seen. once a certain age is reached- particularly for women who are experiencing hormonal changes due to impending menopause- the skin on the face becomes loose and dry. this then tends to droop, which is very visible along the chin and jawline. Another reason is that nothing can really be done to alter one’s chin. if someone doesn’t have a well-defined chin, he can’t undergo any diet or exercise regimen to alter it; unlike obesity which can be addressed by diet, exercise or liposuction. It’s either one will have it that way or have a surgeon improve one’s chin.

Asking for other surgeons who perform the procedure won’t hurt since this would give people options to choose from.

“Greece is a head scratcher,” said Roth.

With a floundering economy, Roth said it is surprising that the country came in second in overall procedures performed.

“A country’s economy and the access to disposable income usually plays a big part in the demand of plastic surgery unless the government pays for it,” said Roth.

But Few said it just goes to show that globalization is affecting the plastic surgery industry in unexpected places.

“There is little room to sit still when we consider innovation, domestic or international,” said Few.

Breast augmentations took the top spot among invasive surgeries in Greece. of note, a reported 592 penis enlargement procedures were performed in 2010, ten times more than the average of other countries that made the list.

The Mediterranean boot came in third with a total of 815,745 procedures performed in 2010. Botulinum Toxin Type a (Botox Dysport) was the most popular procedure performed overall and liposuction was the most common invasive procedure with more than 69,000 reported.

Experts were surprised by Italy’s placement over Brazil, and warned that the data should be taken with caution.

The International Society of Aesthetic Plastic Surgeons issued questionnaires to about 20,000 plastic surgeons throughout the world, requesting the number of surgical and non-surgical procedures they performed in 2010. only 698 came back completed in the allotted time.

“I think clearly we have to take caution with this data, since we don’t know how accurate it is,” said Roth.

Brazil: Home to all people, places and things of hotness. Brazil took up fourth place in the list, and liposuction and breast implants were the most popular invasive surgeries among the population.

“Culturally speaking, Brazilians have no problem with plastic surgery,” said Roth. “If anything, it’s seen as a status symbol to have had it.”

Few said he was surprised Brazil didn’t make it higher on the list, especially because the country has “a number of government-based tax-deduction systems in place to support elective plastic surgery.”

Earlier this year, the government made cosmetic surgeries deductible from income tax, and the discounts are backdated for surgeries as early as 2004. the government believes cosmetic surgery is in such high demand that the additional income to those in the industry will outweigh the money lost to deductions.

Coming in at a close second to its neighbor and fifth overall, Shakira’s home country has a high demand for going under the knife. Liposuction was the most popular procedure, both among surgical and non-surgical procedures, with nearly 90,000 reportedly performed in 2010.

Colombia has become a hot spot for cosmetic tourism, where foreigners travel there to get a nip here and a tuck there.

While there are certainly an abundance of safe options in the countries listed, Roth said it’s important to remember the trusty adage: “If it’s too good to be true, it probably is.”

Cosmetic tourism has become more popular along with higher demand, but Roth warned against going abroad to undergo surgery.

“People go other places because it’s cheap, but you really have to do your homework, even more, if you decide to go abroad to get plastic surgery,” said Roth. “There are absolutely safe procedures being done in other countries, but you have to be very careful. if something goes wrong, a lot of things can go worse when you’re not in your own country with your own doctor.”

The report showed more than 3.3 million reported invasive and non-invasive surgical procedures performed in 2010, but the ASPS reported more than 13 million total procedures performed in the same year. Demand continues to rise as the country slowly makes its way out of economic hardship.

Liposuction showed to be the most in-demand procedure, with breast augmentations coming in a close second.

“Because people are beginning to talk openly about it, we’ve seen over the last couple decades the demand for plastic surgery grow,” said Roth. “People are realizing that it’s more common than most people think and there doesn’t have to be a stigma around it.”

Taiwan rounded out the top 7 List with a total of 221,116 total procedures performed in 2010. Wrinkle reducers like Botox were the most popular among all surgeries performed.

Not only does economy likely play into the Taiwan’s demand for surgery, but trends play a heavy role in the nip/tuck game as well, said Dr. Garry Brody, professor of plastic surgery at USC Medical Center.

“It is more likely how in any given society the local fashion develops in different ways,” said Brody.
