Plastic Surgeons Present New Classification of Fat Grafting Techniques

ARLINGTON HEIGHTS, Ill., Aug 29, 2012 (GlobeNewswire via COMTEX) –As fat grafting becomes incorporated into clinical practice, plastic surgeons propose a new approach to classifying these emerging techniques–emphasizing the need to match the right technique to the right clinical situation, reports a study in the September issue of Plastic and Reconstructive Surgery(R), the official medical journal of the American Society of Plastic Surgeons (ASPS).

“Not all fat grafting is the same,” write ASPS Member Surgeons Daniel Del Vecchio, MD, Boston, and Rod Rohrich, MD, of University of Texas Southwestern Medical Center, Dallas. “Fat grafting, once thought to be a simple technique with variable results, is a much more complex procedure with at least four definable subtypes.”

Renewed interest in techniques using the patient’s own fat for cosmetic and reconstructive plastic surgery has been noted in recent years. in these procedures, fat harvested from one part of the body is transferred to other areas–for example, fat obtained from the thighs using liposuction has been used for breast augmentation and reshaping.

that may sound fairly simple, but in recent years, fat grafting has “exploded into a complex menu of clinical choices.” Plastic surgeons have reported “impressive clinical outcomes” using a wide range of different techniques and additives. the result is “a confusing picture as to which fat grafting technique is best,” Drs. Del Vecchio and Rohrich write.

to clarify this situation, the authors propose a classification of clinical fat grafting techniques. Their classification seeks to match the technique to the individual patient’s situation, based on four factors:

— Method of fat harvesting — Method of cell processing — Method of fat transplantation — Management of the recipient site

For example, they present illustrative cases where smaller volumes of fat were needed to restore loss of fatty tissue in the facial area and reconstructive surgery on a chronic leg wound. in these patients, small amounts of fat were manually harvested from the abdomen using a small syringe.

in contrast, for patients undergoing cosmetic breast augmentation or breast reconstruction after mastectomy, larger amounts of fat were needed. in these cases, fat was harvested from liposuction of the thighs. in these situations, some form of “pre-expansion” of the recipient site in the breast was needed to make room for the larger volume of fat.

Different techniques were also warranted depending on the state of the tissue in the recipient area–for example, inflammation in the chronic leg wound and tissue damage caused by radiation at the mastectomy site. these and other factors may affect the technique used to process fat after harvesting. the most important issues related to fat survival after transplantation may also vary across different clinical situations.

while fat grafting–sometimes called fat transfer or transplantation–is not a new procedure, its development has not been straightforward. At one time, the ASPS opposed the use of fat grafting in the breast, citing possible problems in early detection of breast cancer. More recently, several studies in Plastic and Reconstructive Surgery have reported good results with various fat grafting procedures in a wide range of clinical situations.

Drs. Del Vecchio and Rohrich hope their classification system will provide a useful starting point to maximize the “vast reconstructive and cosmetic potential” of clinical fat grafting. they conclude, “As the true physiology of un-manipulated and stem cell-enriched fat grafts become better delineated, our choices for technical solutions will better fit the clinical problems we face.”

Female Patients Very Happy With Cosmetic Outcome of Breast Reduction

Breast reduction surgery is a very popular procedure among women because of its combined physical and psychological benefits, but are patients happy with the cosmetic results? according to a new research study published in the August issue of Plastic and Reconstructive Surgery, the answer appears to be a resounding yes.

The study, which surveyed 125 breast reduction surgery recipients, gathered information six months after surgery and again one year after surgery.  at the six month marker, a whopping 90 percent of patients rated the cosmetic outcome of their surgery as either “good” or “very good.” a full year after surgery, an enduring 80 percent of patients were still very happy (rating “good” or “very good”) with the cosmetic results of their procedure.

The same study also surveyed the opinions of the cosmetic surgeons and surprising or not, the surgeons were slightly more critical of the final outcome than their patients. at the one year mark about 75 percent of public surgeons rated the final results as either “good” or “very good.” Private surgeons on the other hand were even more critical, with only 60 percent rating the results positively.

According the study, the difference in opinion was related to the evaluation of specific breast features. in general, patients were the most concerned about symmetry between the breasts, while surgeons were overall more concerned about undesirable scarring.

All in all, the take home message of this study appears to be that most women are satisfied with the results of breast reduction surgery, though ratings may decline somewhat over time. however, additional research is probably necessary to help surgeons communicate more effectively the long-term post-surgical expectations of breast reduction surgery.

Does Reality TV Influence Plastic Surgery?

The big question asked by many is whether reality TV shows on plastic surgery are influencing people to have cosmetic procedures done? are these shows the only influence on peoples decision?

In the last seven years there have been five reality shows that follow the lives of the doctors and clients undergoing some form of invasive surgery. they include Extreme Makeover, Bridal Plasty, Dr. 90210, MTV – I want a Famous Face and lastly an American channel Bravo had one called Miami Slice. these shows were broadcast all over the world. the academic journal for Plastic and Reconstructive Surgery have said that four out of five people who seek plastic surgery now have in some way been influenced by these shows. especially those people who were unsure whether plastic surgery was the route to take.

To get a better understanding of the state of plastic surgery have a look at these facts:

• 17 million people undergo plastic surgery every year around the world.
• the top 5 countries doing procedures are America, Brazil, China, India and Mexico.
• Just 10% of surgeries are done on men, versus 90% done on women.
• the numbers of cosmetic surgeries are increasing each year as more and more people are seeking to perfect their bodies.

Make over reality shows may play a role in the change in attitude, in other countries where cosmetic surgeries were not as popular. these shows have a huge fan base and loyal TV viewers. most of the viewers have probably never thought of having breast lift surgery before, but now the idea of it seems easy and may consider having surgery in the future. these shows I have found do skew certain things, especially with regards to patients thinking they can come in and get whatever surgery they want. this is definitely not true; a plastic surgeon will have to determine whether a procedure is right for the patient first.

People in general are influenced in different ways. the most important thing to remember through all this is that it needs to be your personal decision that is done for the right reasons, not because everyone is doing it and not because it is the easy way out.

Breast augmentation is one of the most popular cosmetic procedures performed in the United States. nearly 300,000 women opt for the surgery each year with the procedure rate still steadily growing. While the surgery can bring a women’s form closer to the “aesthetic ideal”, does it really improve patient’s quality of life? Listen, I’ve been saying this for years—breast augmentations are rewarding because they change lives…Conversations about how cosmetic surgery and especially breast augmentation surgery actually changes the lives of my patients is usually met by skeptical eye brow raises for those who have not had cosmetic surgery themselves, of course. I’m delighted that now my clinical experience or trite belief that I’m changing lives one breast at a time is backed up by scientific studies.

A recent study published in the July issue of the journal Plastic and Reconstructive Surgery provides evidence that cosmetic breast implants are a source of long-term happiness and personal satisfaction for women choosing to undergo the procedure.

The study focused on the effect of the surgery on patients’ health-related quality of life and their overall satisfaction. Patients who had undergone breast augmentation were asked to report their satisfaction with their breasts along with their psychosocial and sexual well-being. While they ranked themselves as low on a scale from 1 to 100 in these categorizes prior to having surgery, their scores for feelings of personal satisfaction dramatically increased afterwards and remained high one year later.

New Procedure Makes Bigger Breasts Without Implants

Women who want bigger breasts without implants or scars now have another option, but it can take weeks to get the final results.

Brava, invented by Dr. Roger Khouri of the Miami Breast Center, is a bra-like suctioning device used before and after fat injections into the breasts. Research done in this month’s Journal of Plastic and Reconstructive Surgery showed Brava gave women larger results than fat injections alone.

“They’re large enough for my friends to notice, but they are also natural enough that I can deny it,” said a 33-year-old patient who did not want her name revealed.

She wore the Brava daily for a total of three and half months.

“It has a little bit of suction,” she said. “It just kinds of grabs onto the skin and stays there with a light type of pressure, like a little suction pressure.”

The Brava allows more fat, harvested from the patient during liposuction, to be injected into the breasts by expanding the breast tissue.

“In a small breast, there’s no room to cram a lot of fat cells,” said Khouri. “People historically have tried to stuff it and the capacity is not there. If you crowd them, they all die and you’ll fail. The graft won’t survive.”

The Journal of Plastic and Reconstructive Surgery looked at 81 women ages 17 to 63. It said the 71 who were compliant with Brava wear saw larger breasts, higher fat graft survival and minimal complications.

“We wanted to prove that it actually works,” said Khouri. “There’s no reason not to do if you do it right.”

Doctors say that women who want to see the tissue expansion should wear the suctions cups for ten hours a day for four weeks and then continuously for two days prior to the procedure. The cups also need to be worn 24 hours after the fat is injected.

Woman to receive double arm transplant

A woman who lost her limbs to a flesh eating infection is set to become the first person in America to receive a double arm transplant.

Texan mother Katy Hayes, 43, is hoping to receive a donor’s arms and hands after contracting an infection after the birth of her child two years ago.

Surgeons were forced to amputate all her limbs to save her life and she has since been fitted with prosthetic arms.

However, Fox News reports mrs Hayes is still determined to receive the transplant which is to be carried out in Boston in July.

“I want my life back,” she told a medical team at Brigham and Women’s Hospital, in Boston.

“I want to hold my last child before she’s grown – and she’s already two. if anybody tells me ‘no,’ I’ll just go to the next hospital. no is not an option.”

Her husband Al said in an online blog he is confident his wife would “surpass all expectations” and was remaining upbeat despite complicated procedure ahead.

“In spite of all of this, Katy has kept a positive attitude, and a smiling face,” he said.

Friends and family have raised $200,000 to cover mrs Hayes’ expenses when she undergoes the procedure.

The surgery is expected to take at least 15 hours and require about 40 staff.

Johns Hopkins University plastic and reconstructive surgery chairman Dr. W. P. Andrew Lee said the new nerves must attach to the arms old nerves and grow a very long distance to reach the new hands.

“The nerve in the arm will grow only about an inch a month,” he told the Houston Chronicle.

“It can take a year or two to reach the hand. Functional recovery is less predictable.”

Mrs Hayes will be the second person in the world to undergo a double arm transplant.

A German man was the world’s first recipient of a double arm transplant in 2008.

Facial Rejuvenation in an Hour or Less: Fact or Myth?

There are many advertisements and enticements these days in the media and internet touting the latest and greatest cosmetic surgery procedures.Many of these claim fabulous results without pain and no downtime.One such example of the many procedures I have seen is the so-called ‘lunchtime lift’, in which patients are reportedly able to come in during a lunch break, have a minimally invasive procedure performed in less than an hour, and go back to work with no scars or pain and with results comparable to a complete facelift.Having completed a fully accredited Plastic and Reconstructive Surgery Fellowship training, I can report that such extravagant claims are sadly a myth.Numerous variants of these minimally-invasive facelift procedures have been described, whether with limited tissue undermining, skin excision, or with the use of surgical threads to ‘lift’ the face, but most of these techniques have been shown to be ineffective and with limited, temporary results at best.Many of these false campaigns are advertised unfortunately by self-reported cosmetic surgeons with little or no plastic surgery training of any kind.The above-mentioned ‘lunchtime lift’ was advertised by a family practice doctor where I completed my training.As always, I am a big proponent of seeking consultation with a fully-trained Plastic and Reconstructive Surgeon affiliated with the American Society of Plastic Surgeons.

One area of facial rejuvenation that has really exploded in the past few years that can have significant, immediate tangible effects in less than an hour are the injectable agents.although as a plastic surgeon, I greatly enjoy operating to create aesthetic rejuvenation for my patients, the advent of these injectable agents over the past decade has truly revolutionalized and filled (pun intended!) a niche in the cosmetically-minded consumer market.The patient population that it really appeals to that I personally have seen grow significantly in my private practice in Newport Beach, CA is the mid-thirties to late fifties population who are beginning to see the ravaging effects of aging, sagging facial skin that is exacerbated by the beautiful sunny weather here in southern California.these patients are starting to see wrinkles and creases form in the forehead, glabella (mid-brow), crow’s feet area, cheek folds, and the marionette lines of the lower mouth.although these early signs of aging progressively give patients a tired appearance, they are not quite candidates yet for a full facelift or other surgical procedure.I have noticed many patients in lovely Orange County, especially women, are vibrant, vital individuals who are very proactive in looking and feeling as good as they can.

Enter the injectable agents, which for all intents and purposes are remarkable in that they can be injected in the office in less than an hour with virtually no downtime, negligible pain, and a minimal complication rate.too good to be true?I thought so initially when I was first exposed to these products during my Plastic and Reconstructive Surgery training.however, having been injecting for several years now, I have been continually impressed with the results I am able to achieve for my patients with a very high satisfaction rate.Ultimately, a thorough discussion of the concerns and goals of each patient are essential for an excellent outcome.To the patient and public, the large variety of products available can become overwhelming and confusing; a consultation with your plastic surgeon should elucidate which products can be best for you.

As a brief overview, the injectables can be broadly grouped into the neurotoxin category and the soft tissue fillers category.these products do completely different things that target specific problem areas of the face which is important to understand.The neurotoxins, including the well-known Botox and the newer competitor Dysport, are both purified derivatives of Botulinum toxin type A that aim to paralyze hyperactive muscles of the face that lead to the wrinkles and creases that form with aging.Common areas to inject are the glabella to prevent the “number 11” sign between the brows, the crow’s feet area of the outer eyelid creases, and forehead furrows.The paralysis can require up to a week to take full effect, and results typically last for around 3 months.The art of injecting comes with experience with the key being to provide a softened, rejuvenated look without the unaesthetic stigmata of a ‘frozen’ face.although some practitioners prefer to base their pricing on the number of locations, I think that it is always a good idea to ask exactly how many units are used in each area and base the price per unit to keep it fair, as different dilution methods can be used by various practitioners and the number of units of toxin injected are not always consistent.

The other category of injectable agents is the soft tissue fillers.The most commonly used of these include Juvederm and Restylane which are both hyaluronic acids, Radiesse which is a calcium hydroxyapetite gel, and Sculptra, which is poly-L-lactic acid.There are also other fillers that are not used as often anymore, including Zyderm and Zyplast, which are bovine collagens that require skin testing to rule out allergies before administration, and silicone injections that are not available here in the United States.with time we frequently lose soft tissue fullness from gradual laxity and sagging of our facial skin, giving an aged and weary look.Each variant of filler has a different density and viscosity with its particular range of indications, but they are mainly used to restore lost volume in the face and help smooth out deep creases or wrinkles for aesthetic rejuvenation.Common areas to inject are the tear trough creases (nasojugal folds), depressions around the upper mouth (nasolabial folds) and lower mouth (marionette lines), augmenting the cheek bonesand creating fuller, more voluptuous lips.Temporal hollowing or depressed cheek volume can also be restored with the aid of these fillers.The results are immediate and can last 6-9 months, with some effects frequently seen over a year.
