Presenting on topics covering the correction of facelifts and secondary rhinoplasty, Dr. Rod J. Rohrich presented a series of lectures at the 42nd annual Aesthetic Plastic Surgery Symposium in Toronto last month. Dr. Rohrich, a Dallas plastic surgeon and chairman of the Department of Plastic Surgery at UT Southwestern Medical Center, is an international expert on secondary cosmetic surgery and lectures frequently in teaching symposia around the world.

Dr. Rohrich has carefully developed a method of revising secondary facelifts using what he refers to as “Four R’s” of correction: Resect the scars, Refill the face, Reshape the skin and deep layers and Restore the normal natural contours. his approach is designed to restore the natural and youthful appearance and correct the often unnatural or windswept look that can occur as an unwanted result in some facelifts.

Dr. Rohrich also lectured on secondary rhinoplasty which he considers among the most challenging procedures performed in plastic surgery. “Rhinoplasty is a surgery of millimeters, as the margin of error is very small. it truly epitomizes plastic surgery as a whole because it is a surgery of such great finesse and it takes years to master,” explains Dr. Rohrich. more than half of Dr. Rohrich’s rhinoplasty patients come to see him for revision rhinoplasty after having unsatisfactory results from a previous surgery.

The approach Dr. Rohrich uses in corrective rhinoplasty is very analytical. it involves careful analysis of the nose and deep evaluation of the patient’s expectations and goals. Surgically, Dr. Rohrich prefers to use the open approach in which he will have direct visual feedback on the internal structures of the nose and advocates using autologous grafts, that is, using the patients own tissue when additional supporting structures are needed.

The lectures given by Dr. Rohrich at this meeting included “Rhinoplasty – getting it right the first Time,” “The Lift and fill Facelift – the Role of Fat Compartments in Facial Rejuvenation,” and “Secondary Rhinoplasty – Challenges and the Future.” Dr. Rohrich also presented on how malar fat compartments play a key role in procedures complementary to facelifts such as blepharoplasty.

Most Wealthy Americans Concerned About Maintaining Memory, Eyesight and Weight as They Age; Women Worry About Wrinkles as Much as Health, but Few Go for Cosmetic Surgery or Botox

more than half (53%) of wealthy Americans say that the pursuit ofbetter health and a more youthful appearance have prompted them tospend money on some type of anti-aging regimen, which can range fromsimply maintaining a healthy diet to liposuction.

the tendency to obsess on the effects of aging decreases with age,and is significantly more pronounced in women. Women are more thantwice as likely as men (67% vs. 32%) to have engaged in some form ofanti-aging routine, either presently or in the past. Genderdisparities are also particularly notable when it comes to eatinghealthy foods (76% vs. 55%), getting adequate sleep (58% vs. 41%) anddrinking moderately or not at all (53% vs. 39%).

Memory (59%), eyesight (54%) and weight management (53%) are the topaging-related concerns of both men and women. Women are substantiallymore likely than men to name wrinkles (59% vs. 21%) or skinelasticity (55% vs. 18%) as top aging-related concerns. Females arealso much more likely than men to spend money on upkeep ofappearance, such as coloring their hair (58% vs. 9%), usingover-the-counter anti-aging products (41% vs. 5%), paying for skinresurfacing therapies (10% vs. 2%), receiving injections like Botox(8% vs. 1%) or undergoing cosmetic surgical procedures such asliposuction or a facelift (4% vs. 1%). Men show more concern thanwomen with hair loss, and 5% of wealthy men have tried grafts,transplants or medication to grow back lost hair, compared to 3% ofwomen.

most wealthy Americans hold healthy and realistic notions aboutaging, with 78% saying that a person is “only as old as they feel,”and 71% saying, “age is just a number.” nonetheless, many feel thepressure to look younger: 58% of respondents identify a youthfulappearance as important in achieving professional success, and 68%say that there is more pressure to appear youthful than there was inprior generations. Women are far more likely to feel this pressurethan men (81% vs. 54%), but they seem a bit more pleased with theirprogress in the battle against aging: 72% of women say that they lookyounger than their age, and 62% of men say they do. only 12% of bothsexes say that they look older than their age.
